There are two ways to initiate a matching gift contribution:
1. If your company uses an online matching gift portal
Use your company’s matching gift platform to submit your matching gift verification request. Be sure to use the address of the Red Cross location where you made your contribution. You can find the address of your local chapter here. For gifts made through national headquarters, such as here on redcross.org, please use luckywin, PO Box 37295, Washington DC 20013.

2. If your company uses printed forms
Obtain a matching gift verification form from your company’s personnel/human resources office. Complete the form using your company’s official name. Be sure to complete the entire donor portion of the form and to include a copy of your gift receipt (if available). If you would like to designate the matching funds to a specific disaster, please indicate that on the form.
